At San Fernando Post Acute Hospital, it is our nursing staff that makes all the difference. We are committed to providing you quality medical care with responsiveness, courtesy, and gentleness.
Each of our residents comes to us with very different, and specific, rehab needs. Our entire rehabilitation team is committed to creating individualized therapy programs that directly answer those specific needs. We encourage the resident to achieve their maximum outcomes of independence and ability.
Hospice is a special program dedicated to the needs of the terminally ill with a palliative approach towards care. Our facility contracts with several outside hospice agencies trained in providing quality of life with a focus on the comfort of each individual.
Respite Care is a minimum nursing home stay allowing the routine caregiver, usually a family member, the ability to take time off from their duties while ensuring that their loved one receives the services expected in their absence. At San Fernando Post Acute Hospital we have routine respite care residents. They live in their homes nearby and receive care from a family member that includes medication administration, basic hygiene services, meals, etc. When that family member has a need to be away from home they bring their loved one to us and we assume the caregiving duties. This service is paid for by Medi-Cal with an approved “Treatment Authorization Request” which we will happily submit on behalf of the resident. Any resident can be eligible for respite care with orders from their attending physician and an appropriate payment source. A positive and uplifting atmosphere is vital to the success of healing, recovery, and rehabilitation. San Fernando Post Acute Hospital employs outgoing, friendly professionals to create social programs and activities to fit the residents’ needs. We provide a monthly activity calendar to each resident to keep them informed of upcoming events. Standard monthly events include two outings, one for shopping and one for dining; at least one holiday or special occasion party; a monthly birthday party; cooking class; Bingo, at least twice weekly; as well as daily exercise. Our most special events are held on Mother’s Day, Thanksgiving, and Christmas Eve when we invite family and friends to join our residents and staff in celebrating with a luncheon prepared lovingly by our dietary staff. We also hold a fabulous 4th of July BBQ for our residents and staff with all the trimmings (no families allowed for this one). We also enjoy a Country Breakfast a few times a year where our residents choose their preferences from a menu of items prepared by our very own Department Managers.
We welcome volunteers to our facility to help enhance our activity program. Volunteer applications can be obtained from our Activity Director or our Director of Social Services.
Social services provide essential information, manage requests and concerns, coordinate appointments, maintain contact and open communication between the facility, families and the residents, help in care planning, and manage discharge planning for each resident. We stand up for resident’s rights and strive to make San Fernando Post Acute Hospital their home. We assist with dental, optometry and hearing evaluations upon the request of the resident’s attending physician. In addition to all of this, it is our Director of Social Services who assists our residents and their family or friends through the admission process, seeking to supply the answers to the many questions that arise about staying in a nursing home.
